Aftercare Services

The purpose of our Aftercare Program is to ensure that every client who is successfully discharged from Gibault has a plan to allow for a successful transition back into the home and community. The Aftercare Coordinator uses information gained from working directly with the client, therapist, residential supervisor/case manager, placing agency, and parents to determine what services will be needed in the home community upon discharge.  Prior to discharge, the Aftercare Coordinator helps establish services for each client.  Although these may vary per individual, services may include: finding a job, military services, school enrollment, church involvements, driver's education enrollment, YMCA or YWCA membership, after-school programs, tutoring, home-based services, or family and individual therapy.

After discharge, the Aftercare Coordinator visits the client/family and evaluates the services to determine if their needs are being met.  In addition, the client/family will receive twelve months of aftercare services at no additional charge.

Aftercare is essential because most families are unaware of the social services available to them in their communities.  Aftercare also helps alleviate some of the stressors involved when reintegrating clients back into the community.  Aftercare is an important aspect in the treatment of each and every client that is placed at Gibault Inc.
